Almost two years ago, I send an email asking that question. I got a reply from mrs. Whelan, which said that
There were some "artistic differences" between Michael, the art director, and Meat Loaf and Michael decided not to do the cover.
He really appreciated them asking him, but after the long time he spent on [Stephen King's] the DARK TOWER art, Michael is thrilled to be concentrating more on his personal "gallery" work now. Their idea for the BAT 3 cover was not a direction he wanted to go with his art.
