I imagine it's some kind of hearing problem. Maybe he's damaged his ears after so many years of touring.
Mind you, he doesnt even seem to be aware of the problem. Remember the show in holland in 08 (forgot where exactly), apparently he didnt hit a single note, yet was 100% convinced he hit EVERY note, just a little hoarse or soar...
