Your take on the song is interesting, but in my view the lyrics are a series of rants...sick and tired, prayers never get answered, gods (or the powers that be) ignore us, no light at the end of the tunnel, darkness everywhere etc.

The citizens have bottled up their anger for so long and are now letting Gotham know.

As the song ends they reiterate (more powerfully) that they are as 'mad as hell' and then...the Gothamites fightback! I envisage during and at the end of this scene, villains/goons being captured, beat up etc.
